HB 1401 -- Public School Health Screenings

Beginning in school year 2009-2010, this bill requires public
school students enrolled in the second and eighth grades to
provide proof that they have received a complete physical
examination within the past 12 months.  The bill specifies the
criteria for the examination and requires the Department of
Health and Senior Services to develop a standardized form.
Students whose parents or guardians submit written statements in
certain circumstances may be exempt from the bill.
